W Tertiary Correction doubled


Can anyone refer me to a 2-er in which there are 2 full lines of tertiary white correction by a single WS? I'm almost sure I've seen one by Alex Casa, but I can't find it.
Alternatively the reduced form would do, in which there is a single culminating move by the WS, which is tertiary on 2 lines. (There is Loshinsky 1st Pr Sahs 1960 - any other offers?)
